- Copy of PSA (Birth certificate)
- Copy of HS Diploma / College Diploma
- Copy of Certification (if ALS Graduate)
- Barangay Clearance
- 4 pcs passport size picture with name tag, white background and in formal attire
- 4 pcs 1x1 picture
- 1 pc Long White Folder
- 1 pc Long Red Expanding Envelope
- Copy of Marriage Certificate (for married women)
- Medical Certificate (for DRIVING NC II Only)\

Sa mga nagnanais na maging scholar para sa training na ito, magsadya lamang sa kanilang opisina sa National Highway, Sitio Laguerta, Poblacion 1, City of Calaca, Batangas.
