  • Photocopy of PSA or NSO Birth Certificate
  • Two (2) photocopies of the applicable educational document:
    • Form 137/138 (for elementary undergraduates)
    • High School Diploma (for high school graduates)
    • College Diploma or Transcript of Records (for college graduates)
  • Four (4) passport-size ID photos
  • Two (2) recent 1×1 ID photos with White background, Collar, No nametag, Ears visible and No showing of teeth
  • National Certificate (required only for Trainers Methodology Level I applicants)
  • One (1) A4 brown envelope
