Requirements for enrollment:
- Diploma / TOR (College) / Report card (4th yr HS/Grade 12) OR ALS report card - Photocopy (2copies)
- Birth certificate photocopy (2 copies)
- Additional: If married (woman) - marriage certificate photocopy (2 copies)
- Vaccine card photocopy (2 copies)
- 4 pcs 1x1 & 3 pcs. Passport size ID Picture (Chemical print photo with collar and white background and name tag)
- Brown and plastic envelope. PUT LAST NAME, FIRST NAME, MIDDLE INITIAL AND TRAINING at the back of the envelope.
