- NSO/PSA Birth Certificate (photocopy)
- Marriage Certificate if married (Women only, photocopy)
- Diploma/Report Card (F138)/College OTR (photocopy)
- 4 pcs. 1x1 & 4 pcs. Passport size ID Picture (corporate attire, white background with name tag)
