Qualifications and Requirements:

  • High School Graduate
  • 18 years old and above
  • Barangay Clearance
  • Comelec ID / Certification
  • PSA Birth Certificate
  • Valid ID 
  • Form 137 (High School Graduate)
  • TOR (College Graduate)
  • 6 pcs. 1x1 & 6 pcs Passport Size ID Picture (With collar, Name, and must Print in Chemical)
