Requirements:
- Filipino citizens at least 15 years old
- Medical Certificate and Barangay Certificate
- 3 pcs 1x1 ID Picture
- Photocopy of Birth Certificate or MArriage Certificate, if married
- Previous School Record (HS or College Diploma) or TOR/Form 138/ALS Certificate)
- Photocopy of Government issued ID with 3 specimen signatures
