- 6 pcs Passport size ID Picture with collar and white background (for assessment)
- 1 long brown envelope

Para sa mga gustong maging scholar, i-submit ang iyong application sa PTC-Cagayan, Liban Street, Zone 6, Centro Sur, Gattaran, Cagayan.
