  • Not currently enrolled in school or other education

Requirements

  • PSA Birth Certificate
  • Marriage Certificate, if applicable for married women
  • SF-9 / Report Card for high school graduates
  • Transcript of Records (TOR) for college graduates or undergraduates
  • 3 pieces of recent 1x1 ID pictures
  • 6 pieces of passport-size colored ID pictures
    • With collar
    • White background
    • No name tag
    • Studio shot showing the ears
