Documents to Prepare
Prepare the following requirements:
- One yellow long folder
- Two photocopies of your NSO Certificate of Live Birth
- Two photocopies of your high school or college diploma
- If a diploma is unavailable, two copies of Form 137 or Transcript of Records (TOR)
- Barangay clearance
- Cedula
- Three passport-size ID photos
- Three 1×1 ID photos
- Two 2×2 ID photos
All photocopies must be on long bond paper.
The requirements list specifies an NSO birth certificate. If you have a PSA-issued copy, ask the office whether it is accepted before preparing your submission.
Class Schedules

Day class 15 days
- 8:30 AM–5:30 PM
Evening class 30 days
