Application Requirements
Prepare the following documents:
1. School records — any of the following:
- Photocopy of Form 137 or Form 138
- Photocopy of Alternative Learning System (ALS) Certificate of Rating
- Photocopy of Transcript of Records (TOR) or Certificate of Grades
2. Other requirements:
- Photocopy of PSA/NSO Birth Certificate
- Photocopy of Marriage Certificate, for married women only
- Photocopy of Medical Certificate indicating that you are fit for training
- One 2×2 ID picture with a white background, wearing a collared shirt
