  • 2 photocopies of PSA Birth Certificate
  • 2 photocopies of Marriage Contract, if married
  • 2 photocopies of educational documents such as:
    • Transcript of Records (TOR)
    • Form 137
    • SF10
    • depending on your highest educational attainment
  • 4 passport-size photos with collar, white background, and no name label
