Who Can Apply?
Applicants must be:
- Registered voter of District 1, Parañaque City
- At least a High School Graduate
Documentary Requirements
Prepare the following documents before submitting your application:
- Academic Credential (Form 137, Form 138, Transcript of Records, or Diploma)
- Two (2) Passport-size ID Photos with White background, collar and nametag
- Two (2) 1x1 ID Photos with White background collar and nametag
- Photocopy of Student Permit or Driver's License
- Medical Certificate (Fit to Train/Work)
- Photocopy of PSA Birth Certificate
- Photocopy of PSA Marriage Certificate (for married women only)
- Photocopy of Voter's ID (Registered in District 1, Parañaque City)
- Certificate of Indigency
- Long Brown Expandable Envelope
