- Five (5) A4-size photocopies of one valid ID with three handwritten signatures on each copy
- Two (2) recent 1×1 ID photos with a white background
- Two (2) photocopies of your PSA Birth Certificate
- Two (2) photocopies of your Marriage Certificate (if applicable)
- Two (2) copies of a Certificate of Training or Proof of Prior Knowledge (optional)
