  • PSA Birth Certificate
  • Marriage Certificate (if married)
  • Transcript of Records (TOR) or Copy of Grades
  • Diploma
  • Valid ID with three specimen signatures
  • Three (3) copies each of:
    • Passport-size photo
    • 1×1 photo
    • 2×2 photo
  • One (1) long brown envelope
