- 2 photocopies of Transcript of Records, Form 137, or Form 138
- 1 photocopy of Diploma
- 4 passport-size photos and 2 pieces 1×1 ID photos (white background, with collar and nametag)
- 1 long white folder
- 1 long brown envelope
