- Residente ng Pasig City
- 18 taong gulang pataas
- Nakapagtapos ng sekondarya (At least -High School graduate)
Documentary Requirements:
- Brgy. Clearance
- Birth Certificate (photocopy)
- 2pcs ng 1x1 pic (white background) with COLLAR
- Valid ID (photocopy)
- Itim na ballpen
- Long brown envelope
- Certificate of Employment (kung kasalukuyang nagtatrabaho)

Kaayusan St. Karangalan Village, Brgy. Manggahan, Pasig City. Ito ay bukas
mula Lunes hanggang Biyernes, mula alas-otso ng umaga hanggang alas-singko ng
hapon (8:00 am-05:00pm).
