  • 2pcs. PSA (Photocopy)
  • 2pcs. High School/College Diploma/TOR (Photocopy)
  • 2pcs. Valid ID(Photocopy)
  • Certificate of Indigency
  • Brgy. Health Certificate/ Vaccination Card
  • Brgy. Clearance
  • Marriage Certificate (Only for Married Women)(Photocopy)
  • Long Brown Envelope
