Qualifications:
- Filipino citizens who are at least (18 years old and above)
- Not employed and not in education.
Requirements:
- Photocopy of PSA/NSO (Single)- (6 copies in A4 size)
- Marriage Certificate (if a married woman)- (6 copies in A4 size)
- Photocopy of Student Permit/Driver's License- (5 copies)
- Medical Certificate- (stating physically fit to drive)
- 3pcs. passport size ID picture (TESDA Prescribed)
- Transcript of Records (TOR)/Honorable Dismissal (if college graduate/undergraduate)
- ALS Card & ALS Certificate (ALS Graduate)
- Form 137/138/SF10 (if high school graduate/undergraduate)
- 2pcs. long brown envelope
