ENROLLMENT REQUIREMENTS:
- 2 pcs. 2x2 ID picture
- 2 pcs. Passport size ID (with collar & white background)
- PSA Birth Certificate
- Form 138/OTR/Diploma
- Vaccination Card/Certificate
- Certificate of Indigency/Low income (from Barangay)
