1. Baguio Residence
2. at least fifteen (15) years of age who are not employed,

ENROLLMENT REQUIREMENTS:

• 2 pcs. 2x2 ID picture
• 2 pcs. Passport size ID (with collar & white background)
• PSA Birth Certificate
• Form 138/OTR/Diploma
• Vaccination Card/Certificate
• Certificate of Indigency/Low income (from Barangay)
