- For High School Grad - Form 137/138, Diploma
- For College Grad/Level - TOR
- ALS Graduate - Diploma / Certificate
- Birth Certificate (NSO/PSA)
- If Married - Marriage Contract (NSO/PSA)
- Photocopy of certificate in Practicing Covid-19 Preventive Measure in the Workplace (e.tesda.gov.ph)
