Basic Requirements

- Passport-size photo with white background and collar
- 1x1 photo with white background
- Photocopy of PSA Birth Certificate
- Photocopy of High School Diploma, Form 137, ALS Certificate, College Diploma, or Transcript of Records (TOR), whichever applies
- Barangay Clearance, Police Clearance, or NBI Clearance
- Photocopy of a valid Driver’s License, if available
