This is a good opportunity for qualified Tarlac residents who want to gain practical forklift-operation skills, earn a National Certificate, and prepare for jobs in warehouses, logistics companies, construction sites, manufacturing plants, and other industries that use material-handling equipment.

Who Can Apply?
Applicants must meet the following qualifications:
- Resident of Tarlac Province
- At least 18 years old
- High school, senior high school, ALS, college-level, or college graduate
- Physically and mentally fit for the training
- Able to submit the complete documentary requirements
- Holder of a valid driver’s license (original and photocopy)
Requirements to Prepare
Bring the original documents for verification and submit photocopies of the following:
Educational documents
- Diploma and certified true copy of Form 138, Form 137, or Form 9 for high school graduates
- ALS Certificate for ALS passers
- Diploma and certified true copy of Transcript of Records for college-level or college graduates
- Existing National Certificates, if available
Personal documents
- PSA Birth Certificate
- PSA Marriage Certificate, if applicable
- Original Barangay Indigency Certificate
- Original Medical Certificate
- Voter’s ID/Certification or another valid government ID showing your address
Photos and envelopes
- 3 passport-size photos, white background, with collar
- 4 pieces 1×1 photos
- Long brown envelope and clear plastic envelope
