Requirements

- Photocopy of PSA Birth Certificate
- Photocopy of a valid ID
- Original Barangay Clearance
- Photocopy of Diploma or TOR (High School or College)
- Five passport-size ID photos and one 1×1 ID photo
- White background
- Formal attire
- With name tag
