Requirements
Prepare the following documents when applying:
- PSA Birth Certificate
- High school diploma, Form 137, Form 138, ALS certificate, or college documents, if applicable
- Barangay Clearance
- Police Clearance or NBI Clearance
- Recent ID photos
- Driver’s license, if available
