Requirements

- - Educational document, depending on educational attainment:
- - High School Graduate – High School Diploma or Form 138
- - College Undergraduate – Transcript of Records
- - College Graduate – College TOR or Diploma
- - Barangay Clearance
- - Medical Certificate certifying that the applicant is physically fit
The RTC information also refers to the High School Permanent Record Form 137-A/138 as an accepted school record.
