Requirements for Driving NC II Applicants
Interested applicants should prepare the following:
- 2 photocopies of NSO or PSA Birth Certificate
- 2 photocopies of Marriage Certificate/Marriage Contract, if applicable
- 2 photocopies of any of the following: Form 137, Form 138, Diploma, or Transcript of Records (TOR)
- 2 photocopies of Good Moral Character
- Medical Certificate
- Student Permit/Student License or Driver’s License
- 3 TESDA-prescribed passport-size photos with collar, printed on matte paper
- 2 pieces of 1x1 ID picture
- 1 long brown envelope
Make sure all documents are complete before going to the training institution. Bringing incomplete requirements may delay your application or prevent your registration from being accepted.
