- Photocopy of Marriage Certificate, if married
- 4 pieces of 1x1 ID pictures
- 4 pieces of passport-size pictures with nametag, collared attire, and white background
- 2 pieces ordinary long folder
- Certificate of Indigency from the Municipal Social Welfare and Development Office (MSWDO)
