  • PSA Birth Certificate
  • Marriage Certificate, for married female applicants
  • School credential such as Report Card, Diploma, or Transcript of Records
  • 2 pieces 1×1 ID pictures
  • 2 pieces 2×2 ID pictures
  • 2 pieces passport-size ID pictures
