  • Photocopy of PSA Birth Certificate
  • Photocopy of Marriage Certificate (for married female applicants)
  • Photocopy of Form 137, Form 138, or Transcript of Records (TOR)
  • Four (4) photocopies of one valid government-issued ID bearing three signatures
  • One long brown envelope
