Requirements
Applicants must prepare the following:
- Photocopy of NSO/PSA/Local Civil Registrar-issued Birth Certificate
- PSA Marriage Certificate (for married female applicants)
- Photocopy of Form 138, Diploma, Transcript of Records, or ALS Certificate
- Barangay Clearance, NBI Clearance, or Police Clearance
- Three (3) pieces of 1×1 ID photos (white background)
- Four (4) passport-size ID photos (with collar, nametag, and white background)
- One (1) long brown envelope
- Active email address
