General Requirements
Applicants should prepare the following documents:
- PSA/NSO Birth Certificate
- Marriage Certificate (for married female applicants)
- Transcript of Records (for college graduates)
- Form 138 (for high school graduates)
- High School or College Diploma
- Three (3) copies of 1x1 and Two (2) passport-size ID photo with white background and with collar
