Requirements
Prepare the following documents:
- 1 Long Brown Envelope
- Photocopy of PSA/NSO Birth Certificate
- Photocopy of Transcript of Records, High School Card, Form 137, or Form 138
- Photocopy of Marriage Certificate (for married women)
- Two (2) 1x1 ID Pictures
- Two (2) 2x2 ID Pictures
- Four (4) Passport-size Pictures (white background with collar)
Applicant Qualifications
- At least a High School Graduate
- Must be at least 18 years old by the end of the training period
