Requirements
- Applicants must be 18 years old and above and submit the following:
- Any of the following academic credentials:
- Form 137
- Form 138
- Transcript of Records
- Six (6) passport-size ID photos with white background, collar, and nametag
- Barangay Indigency Certificate (for TESDA Program purposes)
- One (1) government-issued ID (submit six photocopies with three signatures)
- PSA Birth Certificate (photocopy)
- Long brown envelope
