- Birth Certificate (PSA/NSO) - photocopy
- Marriage Certificate (for married) - photocopy
- High School Card / Form 137 (for High School Graduate Only) - photocopy
- Transcript of Records (TOR) (for College Graduate/Undergrad Only) - photocopy
- Passport Size Picture with collar & white background - 2 pcs.
- 1x1 ID Picture - 2 pcs.
- Long brown envelope
