ENTRY REQUIREMENTS:
- Filipino Citizen
- At least 15 years old at the end of training
DOCUMENTARY REQUIREMENTS:
- Photocopy of Diploma / Permanent Record / TOR
- 2 Photocopy of Birth Certificate (PSA/NSO) or Marriage Certificate (if married)
- 5 pcs 1x1 ID Picture
- 3 pcs Passport Size ID Picture (White background, no nametag, corporate attire or with collar)
