- Passport size photo (4 pcs, formal (with collar) white background)
- 1x1 photo (4 pcs, white background)
- Copy of Marriage Contract (for female applicants, kung married)
- Certificate of Completion (Courses, Tesda Online Program)
- Certificate of Completion (Practicing Covid -19 Preventive Measures)
- Certificate of Completion (Working in a Gender-Diverse Environment)
- Brown envelope (Long/Legal)
- Plastic Envelope, Long
