REQUIREMENTS FOR ENROLLMENT:
- The applicant must be a Bacolod City resident
- PSA/NSO Birth Certificate- 5 Photocopies
- Diploma/Transcript of Record (TOR)/ Form 137 or Any School Record - 5 photocopies
- Passport size (3.5cmx4.5cm) with head size (from forehead to chin ranging from 27mm to 31mm colored with white background, shirt/dress with collar, not scanned/edited, not rush, no staple/damaged, paper should not be glossy- 6 pieces
- 1x1 picture- 6 pieces
- All requirements must be submitted enclosed in a long sized brown envelope
