Requirements:
- Filipino Citizen
- At least 18 years of age at the time he/she finishes the training program
- Must not be a current beneficiary of other government educational scholarship or subsidy program for beneficiaries.
- 4 Photocopy of marriage certificate (for married women)
- 4 Photocopy of Birth Certificate (PSA/NSO)
- 4 Photocopy of High School or College Diploma
- 4 Photocopy of Form 137 and 138 (for SHS Graduate)
- 4 Photocopy of TOR (for College Graduate)
- 4 Photocopy of grades(for College level)
- 4 pieces Passport sized ID Pictures (must be White background, with collar shirt, no visible teeth, with name tag)
- 4 pieces 1x1 ID Pictures (must be White background, with collar shirt, no visible teeth, w/ name tag)
- Long brown envelope
