Requirements:
- photocopy of any proof of educational background/attainment
- form 138 (hs card)
- form 137 (hs permanent record)
- transcript of records (tor)
- diploma
- als rating card
- a certification indicating the educational level
- NSO/PSA birth certificate for single; for married: Marriage Certificate
- 2 pieces of passport-size ID picture
- 1 pc of 1x1 ID picture
