- NSO/PSA Birth Certificate (Original & Photocopy)
- College or High School Diploma/ Transcript of Record or Form 137
- Marriage Contract (for Married Woman)
- Barangay Certificate or Clearance
- Vaccination Card (Original and Photocopy)
- 3 pcs 1x1 ID Picture (White Background, Formal Wear, and Quality Picture)
- 3 pcs Passport Size ID Picture (White Background, Formal Wear, and Quality Picture)
NOTE: Provide 2 COPIES for each requirement, put it in a LONG BROWN ENVELOPE, with your FULL NAME (ex. DELA CRUZ, JUAN S.)
How to enroll?
- Choose the desired program (course).
- Check if you are qualified to avail of the scholarship.
- Prepare the requirements needed for enrollment.
- Go to the Executive Excellence of Technical Skills Inc. office located on the 2nd Floor of Sienna's Bldg. Purok 1 Barangay Binanuan Talisay, Camarines Norte. Fill up the learner profile and register to BSRS,
- Wait for the notification from the admin office for the schedule of TIP.
