- 18 years old and above
- At least High School graduate
- Birth Certificate (PSA Copy)
- Valid ID
- CARD/Form 137 for high school graduate
- TOR for college graduate
- Marriage Certificate if married (female only)
- Active Gmail account (for BSRS registration)
- 4 pcs. Passport Size Pictures
- 3 pcs. 1x1 Pictures
