- - Original copy of ALS Certificates (for ALS Passers, and must indicate "High School for old curriculum)
- 2 pcs. 1x1 ID picture with a collar and white background
- At least 18 years old
- At least High School (old curriculum) or Senior High School Graduate
- Barangay Certification (Certificate of Indigency)
