- 3 Photocopies of NSO/PSA Birth Certificate

- Certificate of Indigency
How to Enroll?
Applicants must submit their documents/requirements at San Bartolome Training Center Inc. located in Purok 1 Brgy. Payao Catbalogan City, Monday-Friday 8am-5pm.
