Narito ang mga kailangang requirements:

  • 2 pcs. of 1x1 and 2 pcs. 2x2 ID pictures (white background)
  • Barangay Clearance (Original & Photocopy)
  • Birth Certificate or Marriage Contract, if married (Original & Photocopy)
  • 1 Government Issued ID (4pcs photocopy front and back per pages)
  • For Senior High School graduate: HS Card or Form 137, HS Diploma (Original & Photocopy)
  • For College and Undergraduate: Transcript of Records, Diploma (Original & Photocopy)
  • For Returning/Repatriated Overseas Filipino Worker (OFW) Last contract, Passport (Original & Photocopy)
