- High School Graduate/Senior High Graduate/College Level
- 2 Photocopies of Form 138/Transcript of Records (TOR) and original copy for reference
- NCAE/YP4SC Result
- Certificate of Indigence (Original Copy)
- 2 Photocopies of Birth Certificate (NSO/PSA) (Marriage Certificate if Married)
- 6 Pcs Passport and 3 pcs 1x1 size ID Pictures with Name Tag, white background w/ collar shirt)
- 3 pcs long folder
- Proper attire and 3x4 haircut for boys
